Please Note – Minimum requirements for a candidate to be forwarded to the Clinical Director for consideration are: • Up to date and comprehensive CV with all gaps of 3 months or more explained • Current Referee Reports x 2 In accordance with the Credentialing and Defining Scope of Clinical Practice for Medical Practitioners and Dentists in Queensland Health Policy 2011, we require two (2) referee reports. Referee reports are to be from professional peer referees who are independent of the applicant, with no conflict of interest, and who can attest to the applicant’s clinical performance within the previous 12 calendar months. One of these referee reports should be from the most recent place of employment (or in the case of locums, the most recent locum posting). • Evidence of Vaccination Preventable Diseases (VPD) • Correctly certified ID - Copies must be certified by a Justice of the Peace, Commissioner of Declarations, Barrister, Solicitor or Notary Public. Mackay is a Digital Hospital – ieMR is used for medical records management; we no longer use paper in undertaking patient care Notes, Obs, Prescribe, order tests. All medical officers new to Mackay and ieMR MUST undertake a period of training on the first day of the locum contract. Please contact Mackay Medical Workforce for more information. General information: • AHPRA registration with no restrictions. • Remuneration as per SOA/B45 schedules. PAY RATE NEGOTIABLE • Accommodation & vehicle will be provided. • Criteria for acceptable CV and Referees: o CV – no older than 12 months, must include most recent position held. Gaps of 3 months or more must be explained. An email of explanation can be attached. o References – 1 within 12 months, the other from your last place of locum/employment. References to be signed by the referee or by the agent if verbally taken. • Up to date ALS, PAL (APLS) EMST etc as applicable • Please Note: candidate information will not be forwarded to the Clinical Director without meeting the above criteria. Certified Copies of identification and qualifications As per HR Policy B1 – Recruitment and Selection, applicants are to provide certified copies of their proof of identity documentation and qualifications. Copies must be certified by a Justice of the Peace, Commissioner of Declarations, Barrister, Solicitor or Notary Public.
