An Metropolitan NSW hospital requires a Registrar/PHO in Palliative Care Medicine from 12/02/2025 to 12/03/2025.
Hospital Setting:
1. Accept referrals from other teams and triage them accordingly
2. Review the patient promptly and discuss management plan with specialist for the day
3. Escalate concerns accordingly to specialist
4. Participate in weekly MDT
5. Daily morning hand over and attend consultants rounds
6. Liaise with medical team at PCU and organise transfers to Camden PCU after liaising with specialist at CTN hospital
7. Awareness of opioids and EOL medications
8. Organise PEACH where required
Community Setting:
1. Liaise with community palliative care nursing team regarding medical home visit
2. Liaise with Community Palliative care specialist regarding medical home visit
3. Assessment of patients at home and liaising with Specialist with a management plan
4. Once Management plan is in place then to communicate with nursing and GP
5. Prompt home visit letters and documentation post home visit
6. Escalate an concerns with specialist, nursing staff, GP
7. Transfer patients to hospital if required or organise admission
8. Awareness/competent with opioids and EOL medications
9. Palliative Care experience
